Medical or health grounds – impact assessment statement

If a student's application is to be considered on medical/health grounds, all fields of this statement must be completed by the registered medical/health

practitioner who treated you for the condition. A standard medical certificate is insufficient and must be supported by a completed Medical Impact Assessment Statement by the treating doctor.

The special consideration supporting document is attached below and can be downloaded and shared with your doctor as part of your appointment.
